The Vision, Hearing and Lab/Imaging portions of the program are not available in the following states: AK, CT, FL, IL, IN, KS, MT, NV OR, SC, SD and VT. The Rx program is not available in KS. This plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all healthcare service but will receive a discount from those healthcare providers who have contracted with this discount plan organization.
